    A young mother was beaten to death by her violent former partner after police failed to act on her repeated reports of abuse, investigators said today.
    Casey Brittle, 21, called Nottinghamshire Police 11 times before she was murdered by Sanchez Williams in front of their two-year-old daughter.
    Amerdeep Somal, a commissioner at the Independent Police Complaints Commission (IPCC), said Williams, from Nottingham, was 'well-known to local police for his propensity for violence and threatening behaviour'.

    'In this case it is clear that a number of officers failed to perform to the level expected of them and basic actions, that may have helped others see the full picture of her suffering, were not completed,' the commissioner said.
    'No consideration was given to why Casey was reporting domestic abuse but then subsequently saying that she did not want police help.'

    Ms Brittle died from a series of injuries to her head, including a fractured jaw, cuts and bruising at her home in October last year.


    Their daughter entered the bedroom at least four times while the attack took place and was heard screaming and crying for her mother, Nottingham Crown Court heard in March.
    Ms Brittle was discovered unconscious by a friend and her daughter found asleep in the bedroom at the foot of the bed.

    Williams, 27, was jailed for life after he admitted murdering Mr Brittle.
    Police received allegations of domestic violence and abuse against Ms Brittle between September 2008 and August 2010.

    The police watchdog recorded a number of force and individual errors.


    An IPCC spokesman said: 'As a consequence of the investigation, six officers faced misconduct for their roles in three incidents, while a further four officers have been dealt with through unsatisfactory performance procedures.
    'Aside from failures by individual officers, the investigation identified the lack of a thorough corporate approach to domestic abuse from Nottinghamshire Police.
    'This resulted in a lack of knowledge and support for officers in their attempts to provide victims with the necessary support.'
    The IPCC said in October 2008 the force removed its domestic abuse policy from circulation after recognising it was not fit for purpose.
    No substitute document was put in place until March 2010.
    In addition, the IPCC said the failures were compounded by the fact the force had previously been warned about the same issues.

    In late 2009 the watchdog published the findings of its investigation into how the force had responded to reports of domestic violence concerning Gail Hdili. She had been seriously assaulted by a former partner.
    Assistant Chief Constable of Nottinghamshire Police Paul Broadbent said: 'Whilst awaiting the IPCC’s report, we took steps to completely revise our approach towards responding to, and identifying, incidents of domestic violence and abuse.

    'This involved an overhaul of working practices, specifically in relation to the identification and management of the risks faced by victims.'
    He added: 'We will never know, as the IPCC report sets out, whether we could have saved Casey’s life had we taken a different approach to those 11 incidents involving Casey and her violent partner.

    'What is certain, however, is that we could and should have done more. That is a cause of deep and lasting regret and I have formally apologised to Casey’s mother.'
    Ms Brittle's mother questioned today whether 'one small change' could have saved her daughter as the police watchdog published its findings.
    Victoria Blower said in a statement: 'Losing Casey was the absolute worst day of my life.
    'She leaves behind such a vast breach in our family, words cannot describe the absolute and total devastation of the day she was stolen from us.

    'My beautiful, outgoing, fun, ambitious daughter, friend to many, loved by all who knew her. She was such a special person to us and even now, nearly one year on, the disbelief still lingers.

    'Our family will never forget the fantastic girl that lit up our lives with her gorgeous smile, and her silly sense of humour that still lives on in her little girl.
    'I for one will never recover from losing her, my life has an emptiness that is with me from the moment I wake to the moment I fall asleep.'
    Mr Blower said she knew mistakes were made in dealing with previous attacks on her daughter but said ultimately only one person is responsible for her death.
    She said: 'Maybe one small change in the way things were handled could have saved her, or maybe Sanchez Williams was a time bomb just waiting to explode and nothing that anybody could have said or done was ever going to prevent him from murdering Casey.
    'My greatest hope is that lessons will be learned from what happened to my daughter and that another parent whose daughter is at risk will not have to endure what my family and I have had to.

    'Violence against anyone is not acceptable and should not be tolerated, especially when it is a man using his power and strength against a woman.

    'Nottinghamshire police have taken positive action since Casey's death and I am glad to have been involved in their work to improve the handling of incidents of domestic violence.
    'Although there is no way of knowing whether Casey could have been saved I sincerely hope that in the future a better attitude towards women at risk will ensure their safety.
    'I would also urge people not to close their own eyes and ears if they see or hear somebody experiencing domestic abuse, but to pick up the phone and call the police.
    'You never know it might be your call that saves a person's life. It is too late for my beautiful bubbly girl, but don't let it be your daughter, neighbour or friend who is next.'
